MomCare

MomCare is a program which began December 2001 and is funded by Medicaid. MomCare is specifically designed for moms-to-be who would not normally be eligible for Medicaid but qualify because of an expansion of the eligibility criteria.

MomCare Program

The MomCare Program is for Medicaid eligible pregnant women. The Maternity Care Advisor (MCA) contacts each pregnant woman by phone and provides the following services to every woman being served:

  • Assistance in choosing a prenatal care provider if she does not already have one.
  • Follow-up services to ensure she is able to keep her healthcare appointments.
  • WIC information and assistance in applying, as needed.
  • Referral for Healthy Start services, when needed.
  • Access and referrals to other community resources.
  • Assistance in choosing a pediatrician for her newborn prior to delivery.
  • Instructions on how to obtain Medicaid for her newborn.
  • Information about accessing family planning/birth control services after delivery.
  • Educational information by mail.

The purpose of the program is to help pregnant women select a prenatal care provider and assist them in receiving early prenatal care for a healthy birth. The program also allows for case management, which identifies at-risk women through the Healthy Start Screening process. Please contact us at 772-794-7400 ext. 2710 for more information.
